Perth and Smiths Falls District Hospital
The Perth and Smiths Falls District Hospital is a fully accredited health care facility located on two sites serving a catchment population of approximately 44,000 residents within the tri-county region of Lanark, Leeds and Grenville.
The Perth and Smiths Falls District Hospital was incorporated in 1995 following the amalgamation of the Smiths Falls Community Hospital and the Great War Memorial Hospital of Perth District. The hospital is located within the South East Local Health Integration Network (LHIN#10).
The Perth and Smiths Falls District Hospital has a long history in the provision of high quality health services to its community.

Hospital Foundation
We are happy to announce the successful amalgamation of the Great War Memorial Hospital Foundation and the Smiths Falls Community Hospital Foundation. As of February 1st 2021, the Foundation offices are working as one legal entity – the Perth and Smiths Falls District Hospital Foundation.
The Foundation will continue to operate at both the Perth and Smiths Falls sites and will be open Monday to Friday 8am to 4pm. This amalgamation will allow the best elements of both foundations to be combined and provide a solid organization for future development and continued support by the entire community.
We look forward to continuing the vital work of raising much needed philanthropic dollars for our community hospital, its equipment and the amazing people that work at the Perth and Smiths Falls District Hospital. While the name has changed, the ways to give have not.

Home and Community Care Support Services, formerly called the Local Health Integration Networks (LHINs), are the only organizations in Ontario that bring together health care partners from the following sectors – hospitals, community care, community support services, community mental health and addictions, community health centres and long-term care – in order to develop innovative, collaborative solutions for more timely access to high quality services. By supporting these important partnerships, LHINs are ensuring that Ontarians have access to an effective and efficient health care system that delivers an improved health care results and a better patient experience.
The main roles of the LHINs are to plan, fund and integrate health care services locally. LHINs fund the health service providers and can put local health dollars where they are most needed, based on community priorities.
