You're Invited:  Dying Matters - A Community Conversation with The Hospice Hub

The Hospice Hub, hosted by the Perth and District Community Foundation, invites you to engage in a conversation and share your experience on Dying in Lanark (including Westport and Smiths Falls).

The Hospice Hub is a newly created community-led initiative bringing together key players to enhance the support and care available to all of us affected by serious illness, caregiving, dying, death, loss and grief. The Hospice Hub believes the community plays an essential role alongside health care providers and social agencies to address the gaps in quality and continuity of care at the end of life. Even the best healthcare cannot be everywhere at all times.

This urgent and facilitated conversation will explore how to collectively move forward to build a compassionate community where everyone is supported to live and die well.
